Klinische Schlaganfallstudien

Die klinische Studientätigkeit der Sektion Vaskuläre Neurologie der Abteilung fokussiert sich auf multizentrische Studien zur Akuttherapie und Prävention von Schlaganfällen. Für die Koordination und Durchführung dieser Studien wurde das Studienzentrum Vaskuläre Neurologie eingerichtet. Das Studienzentrum stellt die räumliche und personelle Infrastruktur zur Durchführung derartiger Studien zur Verfügung und ist in der Abteilung Neurologie der Neurologischen Klinik der Universität Heidelberg die zentrale Stelle zur Koordination, Organisation und Realisierung für klinische Schlaganfallstudien. Insbesondere mit den Abteilungen Neuroradiologie und den Kliniken für Neurochirurgie und Gefäßchirurgie sowie mit dem Koordinationszentrum für klinische Studien Heidelberg (KKS) bestehen sehr enge Kooperationen. Gemeinsames Ziel ist es, neue Behandlungsmöglichkeiten für Schlaganfallpatienten zu erproben sowie bereits vorhandene Behandlungsmethoden weiter zu verbessern.  

 

Zu folgenden Indikationen werden aktuell klinische Studien angeboten:

Studientitel Prospective, Open-Label Study of Andexanet Alfa in Patients Receiving a Factor Xa Inhibitor who have Acute Major Bleeding (ANNEXA-4)
Kurzbeschreibung

This is a multicenter, prospective, open-label study of andexanet alfa (also referred to as “andexanet”) in patients presenting with acute major bleeding who have recently received one of the following fXa inhibitors: apixaban, rivaroxaban, edoxaban, or enoxaparin. Attempts will be made to enroll patients on direct fXa inhibitors as well as those on indirect fXa inhibitors, and to limit the percentage of enrolled patients receiving indirect fXa inhibitors to ≤ 20%. A minimum of approximately 110 evaluable patients with ICH will be enrolled in the study. In order to achieve sufficient numbers of ICH patients, enrollment in other subgroups (e.g., patients with non-ICH bleeds) may be capped.

A PHASE 4 RANDOMIZED CLINICAL TRIAL OF ANDEXANET ALFA [ANDEXANET ALFA FOR INJECTION] IN ACUTE INTRACRANIAL HEMORRHAGE IN PATIENTS RECEIVING AN ORAL FACTOR XA INHIBITOR

Kurzbeschreibung This is a randomized, multicenter clinical trial designed to determine the efficacy and safety of andexanet compared to usual care in patients presenting with acute intracranial hemorrhage within 12 hours of symptom onset (from the baseline scan) and within 15 hours of taking an oral factor Xa inhibitor (from randomization). The study will use a prospective, randomized, open label design, as it is unfeasible to blind the Investigator to the treatment assignment given the many potential therapeutic options available under usual care treatment.

Studientitel Early versus Late initiation of direct oral Anticoagulants in post-ischaemic stroke patients with atrial fibrillatioN (ELAN)
Kurzbeschreibung Atrial fibrillation (AF) is the most common cardiac arrhythmia increasing the risk of stroke and systemic thromboembolism and thus mortality and morbidity. Patients with ischaemic stroke and AF are at risk of developing early recurrent strokes with highest risk rates during the first 30 days. Anticoagulation therapy, such as with vitamin K antagonists effectively prevents strokes in patients with AF, however, increases bleeding complications leading to symptomatic intracerebral haemorrhage. Direct oral anticoagulants (DOACs) are at least as effective as vitamin K antagonists in preventing recurrent strokes, but with lower rates of symptomatic intracerebral haemorrhage. Therefore, these new agents are potentially ideal drugs to treat patients with ischaemic stroke related to AF. However, in previous trials comparing DOACs with vitamin K antagonists, therapy was initiated later than 7-14 days after onset of ischaemic stroke. Whether, earlier initiation of DOACs may prevent recurrent stroke without increasing the risk of symptomatic intracerebral haemorrhage remains to be determined.

Kurzbeschreibung To determine whether prolonged ECG recording using either non-invasive continuous ECG recording for 28 days or a subcutaneously implanted event recorder compared to 24-h ECG recording increases the detection rate of paroxysmal AF (pAF) within 6 months after the event in patients with recent TIA. ANDTo determine whether 28-day non-invasive ECG monitoring is non-inferior compared with invasive ECG event recording using a subcutaneously implanted recorder at 6 months.

Studientitel Safe Implementation of Treatments in Stroke
Kurzbeschreibung SITS (Safe Implementation of Treatments in Stroke) is a non-profit, research-driven, independent, international collaboration, with its base at the Karolinska Institute in Sweden. SITS is an initiative by the medical profession to provide safe implementation of stroke treatment in routine clinical practice. SITS vision is to reduce the global burden of stroke; by improving the stroke outcome, preventing recurrent stroke and find the best treatments. This will help the authorities to monitor new treatments and set new guidelines. This will also increase the quality of life for stroke patients and save lives.

Category B. The use of Ticagrelor (Brilique®) and Clopidogrel (Clopidogrel Zentiva®), co-administered with acetylsalicylic acid (ASA), is authorized for the prevention of atherothrombotic events in adult patients with acute coronary syndromes (unstable angina, non ST elevation myocardial infarction [NSTEMI] or ST elevation myocardial infarction [STEMI]), including patients managed medically, and those who are managed with percutaneous coronary intervention (PCI) or coronary artery by-pass grafting (CABG). The use of Clopidogrel (Clopidogrel Zentiva®) is also authorised for prevention of atherothrombotic events (myocardial Infarction, stroke, vascular death) in patients with recent atherothrombotic events or manifest peripheral artery disease. The use of Ticagrelor (Brilique®) and Clopidogrel (Clopidogrel Zentiva®) for carotid artery stenting in the present study is outside the authorised indication

Kurzbeschreibung

The primary aim of this study is to assess the hypothesis that a strategy with low-dose ASA (81-100mg/day) and intensive blood pressure treatment (targeted systolic blood pressure below 120mmHg) with weekly measurements using a home blood pressure measuring device reduces the risk of aneurysm rupture or growth compared with standard care (i.e. no ASA, blood pressure management according to guidelines which usually advise treatment if systolic blood pressure exceeds 140mmHg, and no home device for weekly blood pressure measurements).

The main objective is to perform a RCT to resolve the long-standing management dilemma of antithrombotic stroke prevention in intracerebral haemorrhage (ICH) survivors with comorbid atrial fibrillation (AF). Specifically, it will address the question whether direct oral anticoagulants (DOACs, intervention) provide a more effective option for prevention of ischaemic stroke (IS) and an equally safe option in terms of recurrence of ICH for antithrombotic stroke prevention in survivors of recent ICH compared to no anticoagulation (i.e. no antithrombotic therapy (noAT) or antiplatelet therapy (APA) at Principal Investigator´s discretion).

Kurzbeschreibung Zur Optimierung der Therapie des akuten Schlaganfalls vor dem Hintergrund der aktuellen Behandlungsempfehlungen möchten wir im Rahmen eines prospektiven Registers alle Pateinten mit ischämischen Schlaganfall, die  in Kliniken des Schlaganfall-Versorgungsnetzwerks Rhein-Neckar behandelt warden, erfassen. Hierbei sollen sowohl eine Machbarkeits- und Effizientanalyse von FAST in Hinblick auf eine optimierte Versorgungsinfrastruktur für dir zeitkritischen Schlaganfall-Akuttherapie erfolgen, als auch eventuelle Nebenwirkungen und der klinische Erfolg der Behandlung im Verlauf standarisiert werden.

Studientitel PRediction of Acute coronary syndrome in acute Ischemic StrokE (PRAISE)
Kurzbeschreibung Übergeordnetes Ziel der PRAISE-Studie ist es, mithilfe nicht invasiver diagnostischer Maßnahmen einen Algorithmus zu schaffen, der bei Schlaganfallpatienten die Vorhersage eines akuten Koronarsyndroms erlaubt. Um die Vorhersage eines akuten Koronarsyndroms bei Schlaganfallpatienten zu ermöglichen, sollen nun erstmals systematisch prospektiv, sowohl die Troponinwerte und deren Verlauf als auch die Befunde von EKG, Echokardiographie, GRACE-Score und klinischer Symptomatik berücksichtigt werden. Darüber hinaus soll im Vergleich zu einem stabilen Troponinverlauf geklärt werden, inwieweit ein dynamischer Troponinverlauf ein schlechteres funktionelles und kognitives Outcome bzw. eine erhöhte Mortalität vorhersagt.

PRECIOUS will assess whether a pharmacological strategy to prevent common complications in elderly patients with acute stroke is more effective at reducing the risk of death and improving functional outcome than current clinical practice - recommended in guidelines - of waiting until these complications are manifest before initiating treatment.
